How to Apply for a SCAN Audit | 如何申请SCAN审核

Factories or vendors could request a SCAN audit, below are two ways to request a SCAN audit:

工厂/供应商可以通过以下两种方式自行申请SCAN审核。

Method One: Open SCAN Association Website,  go to the “Suppliers & Vendors” page and click on “Order a SCAN Audit”, fill in all required information in English in the pop-up email and send to SCAN.

方法1：打开SCAN协会网站SCAN Association Website，进入 “供应商 & 中间商”页面，点击“订购SCAN审核”，在弹出的电子邮件中用英文填写所有需要的信息并发送给SCAN。

Method Two: Email SCAN ([e-mail]) to apply for a SCAN audit by providing the information listed below in English.

• Location/Factory Name (Location to be Audited):
• Location Address (Full Address Details):
• SCAN ID (If Applicable):
• Location Country:
• Contact Person Name (First and Last Name):
• Contact Person Email Address:
• Contact Person Phone Number (Include Country code):
• Which SCAN Member(s) requested the audit? Or which SCAN members are you associated with?
• Without an associated SCAN member, this request will be processed as a non-sponsored audit with additional registration fee of $1000 charged. 
• How did you learn about SCAN?
• Additional Comments:

方法2：用英语填写以下信息，并通过电子邮件发送至SCAN ([e-mail])来申请 SCAN 审核。

• 工厂名称（被审方名称）：
• 工厂地址（详细地址）：
• SCAN ID（如适用）：
• 所在国家：
• 联系人姓名：
• 联系人电子邮箱地址：
• 联系人电话号码（包括国家代码）：
• 哪个 SCAN 成员要求进行此审核？或者您与哪些 SCAN 成员相关联？(如果没有关联的SCAN成员，此请求将作为非赞助审计处理，并收取额外的注册费1000美元.)
